Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about October 6, 2021, AV was found outside the locked facility near the unsecured facility parking lot. AV was able to elope the facility through the front doors. The facility doors are able to be opened from the inside when continuous pressure is placed on them for approximately 15 seconds. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
