Findings: Alleged Victim (AV) relies on the facility for his/her care. The facility failed to administer AV his/her thyroid medication, anticonvulsant, and antibiotic medications on or about October 4th through 6th, 2021. AV sustained falls on or about July 28th and 29th, 2021 and no temporary service plan or incident reports were provided. The facility failed to provide service, which is a violation of resident rights, is neglect of care and constitutes abuse.
