Findings: Alleged Victim (AV) relies on the facility for h/h care. On or about October 15, 2021, Alleged Perpetrator (AP2) popped medications for Witness #1 (W1), then was distracted by another resident. AP2 then administered W1 medications to AV not realizing until after checking the computer. AP2 failed to provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to provide a safe medication administration system which is a violation or Oregon Administrative rules.
