Findings: Alleged Victim (AV) is a high fall risk resident. On or about October 20, 021, AV suffered an unwitnessed fall which caused a cut to AV’s forehead. AV had suffered two (2) prior unwitnessed falls on September 1st and August 24, 2021, on one of those falls, AV suffered an injury. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
