Findings: On or about October 19, 2021, Alleged Perpetrator 2 (AP2) failed to administer medications to Alleged Victim (AV) as ordered by AV's physician. AP2's actions is considered neglect of care and constitutes abuse. The facility failed to provide a safe medication administration system which is violation of Oregon Administrative Rules.
