Findings: On or about October 26, 2021, Alleged Perpetrator #2 (AP2) entered the Alleged Victim’s (AV) room to assist AV with changing his/her clothing. After AP2 left AV’s room, AP2 reported to Witness #2 (W2) that while AP2 was attempting to assist AV with changing his/her shirt, AV became resistant and attacked AP2. Shortly after AP2 reported the incident, Witness #3 (W3) discovered AV in the hallway, bleeding from scratches to his/her forearms, which later turned into fingerprint bruises. AP2's actions are a violation of resident rights, are considered neglect of care and constitute physical abuse. The facility’s failure is a violation of Oregon Administrative Rules.
