Findings: According to documentation, the facility and AP3 failed to protect AV from involuntary seclusion; by failing to implement appropriate interventions, and providing adequate supervision and documentation, related to concerns of increased behaviors exhibited by W1. The failure resulted in AV going approximately one month without the ability to visit W1, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
