Findings: On or about October 28, 2021, Alleged Victim (AV) and Witness 1 (W1) had a resident-to-resident altercation that caused injury to AV when W1 slapped AV on the right side of AV's face. An investigation determined that W1 has a history of aggressive behaviors/resident altercations. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP22-00444 $375.00 fine assessed
