Findings: The Alleged Victim (AV) was care planned to receive three showers per week. On or about October 31, 2021, AV did not receive his/her shower as care planned, placing AV at risk for harm. The facility's failure to ensure enough staff were working to meet the needs of the residents is a violation of resident rights, is considered neglect of care and constitutes abuse.
