Findings: Witness #7 (W7) has a history of inappropriate sexual behaviors. On or about October 28, 2021 and November 1, 2021, W7 was observed being sexually inappropriate with the Alleged Victim (AV). The facility failed to implement interventions to keep AV safe from W7's inappropriate behaviors, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
