Findings: On or about November 2, 2021, The Alleged Victim (AV) left facility without assistance and was returned to the facility by Law Enforcement due to AV showing signs of confusion and cognitive decline. Based on facility documentation and interviews, facility failed to properly assess AV with having a cognition diagnosis and being a risk for wandering when AV moved into the facility on November 1, 2021. The facility's failure to properly care plan placed AV at risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP23-00228 $375.00 fine assessed
