Findings: In between the timing of September 24, 2021 and November 5, 2021, Hospice was called to the facility to assist with catheter care for the Alleged Victim (AV), due to the catheter being plugged or blood in the catheter bag. On multiple occasions, AV's catheter was plugged, and not releasing urine, causing a urine backup in AV's bladder, causing unreasonable discomfort and potential for infection. The facility failed to provide appropriate catheter care to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
