Findings: The facility failed to follow the Alleged Victim's (AV) care plan to keep him/her away from Witness #1 (W1). AV and W1 engaged in an altercation where AV yelled at W1, W1 then grabbed AV's arm and scratched him/her. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
