Findings: The Alleged Victim (AV) was care planned with aggressive behaviors and was to be redirected when agitated. On or about November 4, 2021, Witness #1 (W1) and the AV had a non-injury altercation where both residents hit each other. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
