Findings: The Alleged Victim (AV) is a known fall risk and has suffered multiple falls. Between July 2021 and November 2021, AV was found on the floor by staff 8 times after AV had been ambulating or transferring by him/herself. The only interventions noted for AV's fall prevention was frequent checks and to assist AV with toileting, which were not effective in reducing AV's falls, which resulted in skin tears and bruises. The facility's failure to properly care plan to reduce AV's risk of falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
