Findings: Witness 1 (W1) has a history of agitation, aggression, antagonizing others, and going into other resident rooms. Interventions in place for W1 is to not be in other resident’s rooms. W1 and Alleged Victim (AV) have history of prior disagreements. On or about November 06, 2021, W1 was verbally and physically aggressive towards staff and AV. W1 entered AV room and was verbally aggressive towards AV. W1 then went outside around to AV window and was verbally aggressive to AV, resulting in emotional distress for AV. The facility failed to appropriately care plan and implement reasonable interventions to address W1 ongoing behaviors which is a violation of resident rights, is considered neglect of care and constitutes abuse.
