Findings: Witness 1 (W1) has a history of being aggressive physically and verbally with staff and other residents. On or about November 3, 2021, W1 was agitated and aggressive all day. W1 was trying to take a blanket from AV and punched him/her when AV didn’t give it up. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP22-01219 $375.00 fine assessed
