Findings: On or about November 14, 2021, the Alleged Victim (AV) was found on the floor of his/her room with Witness #1 (W1) standing over him/her. AV stated that W1 attacked him/her and pulled him/her out of the bed. AV stated he/she had back pain after the incident. W1 is known to have behaviors and aggressive actions towards staff and residents. The facility failed to properly care plan for W1, leaving other residents at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
