Findings: The Alleged Victim (AV) is a known fall risk and between August 2021, and November 2021, AV suffered 10 falls, resulting in pain and injuries from bruises to lacerations and fractures. On or about November 15, 2021, AV fell from bed, resulting in a fractured nose and a laceration to his/her face, which required stitches. This fall was the third fall for AV within 30 days. The facility's failure to ensure AV was safe from falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-00464 $500.00 fine assessed
