- Findings
- On or about November 15, 2021, the Alleged Victim (AV) and Witness #1 (W1) engaged in a physical altercation where W1 slapped AV and AV punched W1. There were no injuries, however, AV was placed at risk for harm. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
