Findings: The Alleged Victim (AV) has orders for his/her blood sugar checked every morning. AV had at least three instances where his/her blood sugar was not checked by staff, placing AV at risk for harm. On or about November 8 and 9, 2021, AV missed 3 doses of ordered medication, placing AV at risk for harm. The facility's failure to provide a safe medication administration system is a violation of resident rights, is considered neglect of care and constitutes abuse.
