Findings: The Alleged Victim (AV) has a known history of falls. AV suffered seven (7) falls during the month of September, including a fall on September 24, 2021, in which AV sustained an injury to the back of his/her head and was taken to the emergency room to be assessed and was found to have a contusion on his/her head. The facility failed to care plan appropriately and implement person centered interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
