Findings: The facility failed to assess, intervene, and monitor when the Alleged Victim (AV) experienced a change of condition. On or about November 16, 2021 it was reported that AV had developed skin issues, no monitoring or interventions were done for approximately three days, putting AV at risk of harm and causing unreasonable discomfort. The facilities failures are a violation of resident rights, are considered neglect of care and constitute abuse.
