Findings: The Alleged Victim (AV) is known to elope the facility, and had previously eloped at least 6 times. One on one supervision was put into place to avoid AV leaving the facility unattended. On or about November 9, 2021, AV left the facility unattended and was found approximately a block away from the facility in an unlocked vehicle. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
