Findings: Witness #1 (W1) has history of wandering around the facility and into Victim (AV) room. On or about November 26, 2021, W1 wandered into AVs room and hit AV in the face. On or about November 30, 2021, W1 wandered into AVs room and a physical altercation occurred, W1 hit AV in the face leaving a red area to the right of AV lower lip and a bruise to AV right arm. The facility failed to properly plan care and provided interventions appropriate to mitigate W1 wandering and aggressive behaviors towards AV, which is a violation of resident rights is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-00426 $375.00 fine assessed
