Findings: Between the timing of November 25, 2021 and November 28, 2021, an unknown person had removed the Alleged Victim's (AV) bag from his/her room. AV, staff and family reported that AV's bag was missing from his/her room to Witness #1 (W1) on November 28, 2021. W1 failed to complete an internal investigation of the matter or report the incident to Adult Protective Services. AV's bag was returned to his/her closet, however, his/her money and check book were missing. The money and check book were taken by an unknown individual and this person is responsible for theft of property, which is considered financial exploitation and constitutes abuse. The facility failed to protect AV's property from theft. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
