Findings: The Alleged Victim (AV) is known to have behavioral issues, which are usually preceded by AV rolling up and down the hall in his/her wheelchair. On or about December1, 2021, AV was rolling up and down the hall and approached Witness #1 (W1). The two yelled at each other then W1 hit AV. Both residents have a history of altercations and the facility should have foreseen this incident. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP22-00400 $338.00 fine assessed
