Findings: On or about November 25, 2021, Alleged Victim (AV) and Witness 1 (W1) engaged in a resident-to-resident altercation which caused injury to both AV and W1. An investigation determined that both W1 and AV have a history of resident-to-resident altercation and on the day of the incident facility failed to provide adequate staffing to ensure W1's care needs were met. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
