- Findings
- Alleged Victim (AV) relies on the facility to ensure a safe environment. Witness 1 (W1) has a history of resident-to-resident altercations. The facility failed to follow W1’S care plan to redirect from noise to avoid agitation and provide appropriate staff to meet the needs of residents, resulting in AV being hit by W1. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
