Findings: The Alleged Victim (AV) was a known fall risk. AV was care planned to have safety checks to ensure his/her safety. On or about November 28, 2021, AV fell in his/her room and was found by a staff member on the floor. AV did not suffer any injury, however, AV was placed at risk for serious harm by not having safety checks as scheduled. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
