Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about, November 28, 2021, AV experienced a fall, which resulted in back and shoulder pain. After the fall, AV pushed the call button and waited over twenty-six minutes for care staff to respond. The facility failed to provide a safe environment by failing to provide adequate staffing, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP22-00394 $250.00 fine assessed
