Findings: On or about December 9, 2021, Alleged Perpetrator 2 (AP2) wrongfully physically restrained Alleged Victim (AV) when AP2 held AV's hands down while Alleged Perpetrator 3 (AP3) used physical force against AV to get a napkin out of AV's mouth causing AV unreasonable discomfort and pain. An investigation determined that AP3 used physical force against AV when AP3 grabbed AV's cheeks in one hand trying to open AV's mouth and then forcefully pushed AV's head back and chin down to try and get AV to open his/her mouth while AP2 restrained AV's hands. AP2's actions is considered wrongful restraint which constitutes abuse. AP3's actions is considered physical abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
