Findings: Witness 1 (W1) is care planned with aggressive behaviors and frequent agitation and had an increase of altercations prior to incident. On or about December 11, 2021, Alleged Victim (AV) and Witness 1 (W1) had an injury altercation which resulted in AV getting lacerations to his/her arms and face. The facility's failure to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
