Findings: On or about December 11 ,2021, the Alleged Victim (AV) wandered out of locked Memory Care unit and was found by a maintenance person wandering in the lobby of the facility. AV had been known to exit seek, however, got out of the unit. AV being outside of the locked unit places AV at risk for harm. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP22-00411 $375.00 fine assessed
