Findings: Alleged Victim (AV), Witness 1 (W1), and Witness 3 (W3) have history of prior resident to resident altercations. W1 has history of and is service planned for wandering into other residents’ rooms. W3 service plan indicates facility will keep him/her at a safe distance from other residents when he/she enters common areas. On or about November 01, 2021, W3 was watching TV with other residents. W1 entered AV room causing W3 to try to protect AV from W1. W1 and W3 had a physical altercation. The facility failed to follow the care plan which is a violation of resident rights is neglect of care and constitutes abuse.
