Findings: The Alleged Victim (AV) relies on the facility to ensure a safe environment. Witness 1 (W1) has a history of resident-to-resident altercations. On or about December 16, 2021, AV sustained a bruise from W1, during the altercation in the TV room. The facility failed to ensure a safe environment for AV. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
