Findings: The facility failed to follow Alleged Victim's (AV) medical order to be re-positioned every two hours as needed for pressure ulcer relief and facility failed to apply AV's barrier cream as prescribed. Based on facility documentation and interviews, AV receives assistance with his/her Activities of Daily Living by both facility care staff and private caregivers. The facility failed to properly care plan for AV's care needs by failing to provide oversight and coordination of care with both facility care staff and private caregivers in which the facility is ultimately responsible for AV's overall care and well-being. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
