Findings: The facility failed to provide appropriate services and care plan according to the Alleged Victim’s (AV) needs. The Alleged Victim (AV) had approximately twenty-three (23) documented falls between February 2, 2021, and December 8, 2021, some resulting in injury, a 25.4-pound weight loss in six (6) months, and on or about December 24, 2021 AV had a severe infection in the lungs, bed sores on AV's bottom and heels and a genital injury. The facility failed to provide appropriate services to respond to AV’s change of conditions, which caused repeated unreasonable discomfort, is a violation of resident rights, is considered neglect of care and constitutes abuse.
