Findings: The facility failed to ensure supervision and staff support regarding known behaviors related to the Alleged Victim (AV) and Witness 1 (W1). An incident occurred between the two where W1 attempted to enter AV's room, resulting in a physical altercation, putting the AV at risk of serious harm and unreasonable discomfort.
