Findings: On January 3, 2022, Alleged Victim (AV) was admitted to the facility as a new resident without a care plan in place to direct AV's care needs. A day after admission, AV was found on the floor, naked and bleeding and AV was sent out to the hospital for treatment. Based on interviews and facility documentation, it was determined that AV's care plan was initiated on January 6, 2022, where it was documented that AV required physical assistance with dressing, transfers, mobility, grooming, and toileting. The facility's failure to properly care plan for AV's care needs at time of initial placement caused an unsafe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
