Findings: The facility failed to follow Alleged Victim's (AV) care plan to complete safety checks every two hours due to his/her risk of falls. On or about January 10, 2022, AV was found deceased, after an unwitnessed fall, and had not been checked on for approximately 12 hours. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
