Findings: The Alleged Victim (AV) is a known fall risk. On or about January 14, 2022, AV was found in his/her closet sitting on his/her bottom. Later in the day, AV was noticed to have a swollen knee and was sent to the emergency room and diagnosed with a fractured knee. The facility failed to properly care plan to reduce the risk of falls suffered by AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
