Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about January 07, 2022, AV pushed call light pendant nine (9) times with no response from the facility. AV was found on the flour when staff delivered a room tray to AV. AV was transported to the hospital for a head injury requiring two staples. The facility failed to answer call light in a timely manner, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP22-00718 $1500.00 fine assessed
