Findings: The alleged victim (AV) is care planned to receive nail care and cleaning services by the facility. On or about December 25, 2021, AV was found to have overgrown nails and their room was not picked up resulting in unreasonable discomfort. The facility failed to follow AV's care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
