Findings: The facility failed to provide appropriate services according to Alleged Victim (AV)'s needs, relating to his/her change of condition, number of falls with lack of interventions, keeping his/her room free of clutter, and AV's known hesitation to call staff when needing assistance. AV experienced approximately thirty-one unwitnessed falls from approximately November 9, 2021, and January 24, 2022, which resulted in repeated unreasonable discomfort, as a result of some head injuries and skin tears. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
