Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about November 11, 2021, AV is believed to have sustained a fall. AV was found down on the floor after staff heard AV calling out for help from their room. AV was found with blood coming down their neck and ear, he/she received surgery in order to have their ear reattached. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP22-01445 $1500.00 fine assessed
