Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about January 25, 2022, AV attended a facility outing to a local store. Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3) did not complete a head count prior to leaving the store. At 1:30 pm, a store employee notice AV wandering and confused and so contacted the facility and staff returned to pick AV at 2:45 pm. The facility, AP2 and AP3 failed to assure resident was safe during the outing, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP22-00661 $500.00 fine assessed
