Findings: The Alleged Victim (AV) is care planned for dehydration and is a high fall risk. On or about January 19, 2022, AV had a fall and was sent to the hospital where AV was diagnosed with fractured ribs and fractured vertebrae. Based on interviews, facility documentation and hospital documentation, it was determined that the facility failed to properly assess, intervene, and monitor Alleged Victim (AV) when he/she experienced a change of condition which resulted in AV suffering pain and unreasonable discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
