Findings: Alleged Victim (AV) has a known history of falls. On or about February 2, 2022, AV had a fall while attempting to transfer from the bed into his/her wheelchair. AV sustained an injury from the fall and had a hip/pelvic fracture. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
