Findings: The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs, relating to his/her change of condition, number of falls with lack of appropriate interventions. AV experienced approximately six falls from approximately December 8, 2021, and February 6, 2022, resulting in skin tears and hip injuries, which caused AV repeated unreasonable pain and discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
